You’ll need
Licensure
✓Valid LPN license in the state(s) of practive including eligibility under an NLC multistate license required .
✓Iowa Board of Nursing (IABON)
✓Dual licensed positions: Team members must obtain and submit proof of dual licensure within 90 days of hire/transfer.
Certifications
✓Illinois Department of Financial & Professional Regulation (IDFPR)
✓Wisconsin Department of Safety and Professional Services (WIDSPS)
✓Proof of application is acceptable during this period – required within 90 days of hire.
✓Must possess and maintain current Basic Life Support (BLS) certification through the American Heart Association required .
✓National Institute of Health (NIH) Stroke International
One (1) year of previous CNA/PCT experience preferred.
National Institute of Health Stroke Scale-English preferred:
Education
✓Graduate of accredited LPN program required .

Support safe care transitionsSupport safe care transitions by maintaining accurate , real-time documentation, participating in hand-offs, and communicating patient progress with the healthcare team
