LPN - Clinical Care Coordinator II - FPG Centralized Clinical Services

DepartmentCentralized Clinical Services Job SummaryThe Clinical Care Coordinator II demonstrate the competency to provide patient care according to the age-specific population needs and assist in the management of patient care in the physician office setting under the direct supervision of a provider. This position is with FPG Nurse Triage Team - will work at FPG's Centralized Access Center on University Parkway in Sarasota.

The LPN will take calls from established FPG patients and follow protocols for patient symptoms and offer guidance based upon strict adherence to established protocols. The goal is to provide excellent patient care and provide real time clinical guidance. This team will work closely with the Clinical Excellence and Operations team to ensure seamless patient care experiences in collaboration with providers and develop effective communication with the provider office care teams.

Required Qualifications- Require Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date.- Require graduate of an accredited school of nursing or a diploma program.

- Prefer one (1) year of clinical experience in outpatient physician office practice.- Prefer experience with an electronic practice management and electronic health record.- Prefer demonstrated knowledge of electronic practice management and electronic health records software along with skill in all Microsoft office software programs (Word, Excel, Outlook, and Power Point).
