About the role

In this role, you'll leverage your clinical expertise and communication skills to coordinate seamless transitions from hospital to home while ensuring patients receive the care and support they need. This role is Monday through Friday 8am-4:30pm with some 4 hour weekend days and holidays, depending upon the needs of the department.

What you’ll do
Collaborate with referral sourcesCollaborate with referral sources, discharge planners, case managers, and providers to coordinate HomeHealth services
Review patient information andReview patient information and obtain required demographics, insurance, and clinical documentation
Meet with patients andMeet with patients and families to assess home care needs and support safe discharge planning
Obtain and maintain consentsObtain and maintain consents, authorizations, Advance Directive information, and physician orders
Coordinate with intake, schedulingCoordinate with intake, scheduling, and clinical teams to ensure timely start of care
