The Patient Navigator II plays an integral part for ensuring patients undergoing complex surgery have the opportunity to follow a best practice pathway pre and post-procedure and work towards optimal results. They will also be expected to serve as a liaison throughout the facility and in the community regarding services provided for patient optimization and support. The Patient Navigator II will support the Clinic Manager/Certification Coordinator by collecting key information for the dashboard and assisting in developing and tracking all process improvement initiatives.
Graduate of an Accredited Program in Practical Nursing Licensed Practical Nurse in State of Ohio (LPN), Basic Cardiac Life Support (BCLS) Minimum of 5 years experience working in a medical field. Familiarity with one or more years in surgical practice. Experience with musculoskeletal anatomy and/or treatments.
