✓Must have current Indiana LSW or LCSW license.

Qualifications
✓Must have valid drivers license and proof of insurance.
✓Demonstrates thorough working knowledge of social work process, community agencies, services, and resources available to assist patients and families in the resolution of emotional , social, and financial difficulties related to change in patient's health status.
✓Requires general knowledge of the normal growth, development, related behaviors, needs, comfort, and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care.
✓Requires sound analytical and organizational skill to assess patient's needs, plan and coordinate appropriate therapeutic intervention and to manage case load to achieve efficiency and productivity.
✓Must have good verbal and written communication skill.
Minimum of one (1) year's experience in health care with experience in hospice or palliative care preferred.
