Licensure
✓Must have current Indiana LSW or LCSW license.
Certifications
✓Addendums are required for 0153 Nurse Leader, 0249 Registry RN, 0252 Registered Nurse, 0235 Advanced Registered Nurse, and 0236 Expert Registered Nurse.
For those employed to work in the outpatient palliative care team, Oncology Social Worker ‐ Certified (OSW‐C) or Advanced Palliative Hospice Social Worker ‐ Certified (APHSW‐C) preferred.
Education
✓Must have a Master's degree in Social Work (MSW).
Qualifications
✓Demonstrates thorough working knowledge of social work process, appropriate clinical interventions, community agencies, services, and resources available to assist patients and families with emotional and social difficulties related to change in patient’s health status.
✓Requires general knowledge of the normal growth, development, related behavioral, needs, comfort and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care.
✓Requires sound analytical and organizational skills to assess patient’s needs, plan and coordinate appropriate therapeutic interventions and to manage caseload to achieve efficiency and productivity.
✓Must have good verbal and written communication skills.
Minimum of one (1) year experience in health care with experience in palliative and/or hospice health care preferred. Prior clinical and/counseling experience preferred.
