$10.00 - $500.00 per hour Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. The Nurse Care Manager is an Illinois licensed Registered Nurse who provides care coordination services to Rush patients through a diverse role as a clinician, educator, care coordinator and advocate. The Nurse is an expert in chronic disease management, prevention, health maintenance, and clinical evaluation of medical and psychosocial needs. Provides timely telephonic outreach to discharged patients in order to better engage them with their own care.

- Bachelor’s Degree in Nursing.
- Current RN Licensure in the State of Illinois.
- Two years' experience as a health care provider for the neonate, pediatric, adolescent, adult and /or geriatric patient, and expert knowledge in case management, discharge planning, social service, and criteria application.
- Exceptional communication skills, both written and oral.

• Three years of progressive nursing experience in hospital, ambulatory or home health setting with a focus in disease or case management.
1. Facilitates communication with patients and care team, coordinates referrals, and promotes optimal allocation of available resources. 2. Adheres to evidence-based treatment guidelines in facilitating care for chronic care/ disease management. 3.
Contributes to documentation of individualized care for identified patients, focusing on specific treatment goals. Assesses progress toward goals based on clinical judgment, review of patients’ self-monitoring tools and trends in clinical data. 4. Assesses and monitors adherence to outreach and problem-solve intrinsic barriers to effective patient self-management of chronic conditions (i.e. education level, language barriers, etc.). 5.
Identifies extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy coverage) and works with patient, care team and other available resources to alleviate barriers. 6. Provides educational materials and resources to patient and family that are in a format which is specific to the patient/family learning abilities and language preference. 7. Refers patients/families to self-management support programs as needed. 8.
Assists with the timely follow-up and coordination of care for patients discharged hospital setting requiring complex care management. 9. Communicates with internal and external care providers (specialty consultants, home health agencies) to ensure safe and effective care management. 10. Analyzes clinical data to track patient outcomes to determine if case management has improved patient status. 11.
Triages patient phone calls for acute patient issues and counsels accordingly. 12. Participates in medical home quality improvement initiatives; activities may include data collection, chart review, interdisciplinary collaboration, analysis of patient data and inter-professional staff meetings. 13. Performs related duties as required.
