We are seeking a Nurse Practitioner with cardiology experience, preferably in heart failure, to provide care in an outpatient transitional care heart failure clinic. The Nurse Practitioner will manage patients for approximately 30 to 45 days in collaboration with their primary care and cardiology providers. This position will focus heavily on assessing and managing volume status and initiating and titrating guideline-directed medical therapy.
The successful candidate must be able to work effectively within an interdisciplinary team and communicate patients’ needs clearly and promptly. Key Responsibilities Clinical Care Perform comprehensive histories and focused physical examinations for patients with acute and chronic heart failure. Diagnose, evaluate, and manage heart failure in accordance with current clinical guidelines, including ACC/AHA and HFSA recommendations.
Initiate, titrate, and monitor guideline-directed medical therapy, including beta-blockers, ACE inhibitors, ARBs, ARNIs, SGLT2 inhibitors, mineralocorticoid receptor antagonists, vasodilators, and other evidence-based therapies. Order and interpret diagnostic testing, including laboratory studies, ECGs, echocardiograms, and imaging, as clinically indicated. Assess and manage volume overload using appropriate diuretic strategies, including loop diuretics, thiazide diuretics, and carbonic anhydrase inhibitors.
Refer patients for IV diuresis, specialty consultation, or a higher level of care when appropriate, ensuring a thorough handoff to the clinicians assuming care. Review available patient monitoring data and respond appropriately to changes in patients’ conditions. Care Coordination and Team Collaboration Collaborate closely with cardiologists, primary care providers, electrophysiology specialists, advanced heart failure and transplant teams, pharmacists, social workers, and case managers.
Ensure timely follow-up after hospital discharge, ideally within seven days and no later than 14 days following discharge. Identify social determinants of health that may affect patients’ ability to manage their conditions and coordinate appropriate interventions with the multidisciplinary team. Facilitate seamless transitions from inpatient to outpatient care to help reduce 30-day hospital Implement evidence-based protocols, clinical pathways, and standardized orders to ensure high-quality, consistent care.
