Licensure
✓Eligible for licensure by the Texas State Board of Pharmacy (TSBP) Upon Hire and
✓Texas license with TSBP within 30 Days and
Certifications
✓(BLS) BASIC LIFE SUPPORT certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire and
✓(ACLS) ADV CARDIAC LIFE SUPPORT course accredited by the American Heat Association (AHA), or American Red Cross (ARC) required for only inpatient positions prior to or within 6 months of employment.

Education
✓Both PGY1 and PGY2 from an ASHP-accredited residency program
✓Bachelor's Degree of Science degree or PharmD from ACPE accredited school of pharmacy or
✓Doctor of Pharmacy (PharmD) degree from an ACPE accredited school of pharmacy
✓5 years progressive clinical practice experience within specialty with PGY1 from an ASHP-accredited residency program or
Qualifications
✓PGY1 with 5 years of progressive clinical practice experience
✓8 years progressive clinical practice experience within specialty.
✓Computer skills, data entry experience
✓English (speaking, reading, writing). Good general health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc.
