The Palliative Nurse Navigator in collaboration with the palliative care providers/clinicians providing quality and specialty palliative care to patients and their loved ones. This care includes: ongoing assessment of patient and family needs including the 8 domains of palliative care and connecting patients and families to available resources within the health system and in the community. Qualifications Education, Knowledge, Skills and Abilities Required: Graduate of licensed school of nursing; RN with at least 5 years of clinical experience in a relevant clinical specialty.

Proficient computer skills- proficiency in Google, Microsoft, EPIC Strong organizational, time management, critical thinking RN skills. Ability to work independently and exercise sound judgment in interactions with providers, Clinicians, patients, and their families. Excellent written and verbal communication skills.

Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Education, Knowledge, Skills and Abilities Preferred: BSN Licenses and Certifications Required: NJ State Professional Registered Nurse License. AHA Basic Health Care Life Support HCP Certification.
