Serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies. Performance: Position Specific

Dispenses medications and IV medications for patients ranging in age from neonatal through elderly adults in accordance with local, state, and federal laws and hospital policies and procedures. Interprets medication orders and transcribes into the computerized patient medication record accurately. Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols Participates in the orientation of all new employees; provides in-services education to pharmacy and hospital staff; assists in preparing performance evaluations as required.

Assures accurate and complete labeling of drugs distributed from pharmacy; maintains records of transactions to assure control and accountability for all drugs. Participates in quality improvement and medication use reviews in accordance with department and medical staff approved criteria. Sustains the formulary by minimizing non-formulary procurement, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.

Provides clinical consultations and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed. Documents all clinical activities and interventions accurately and completely. Performs other duties as assigned.

Qualifications Education Required High School Diploma, GED, or equivalent Bachelors Degree in Pharmaceutical Science or related Doctorate degree in Pharmacy

Preferred Two years of experience in pharmaceuticals Certification/Licensure: Required Registered Pharmacist by the Texas State Board of Pharmacy
