Needs to function independently and be a good team member. Ability to work 2 weekend shifts per month & one holiday per year in Per Diem PT - Weekend job title.

This position uses a variety of physical therapy techniques to restore individuals to maximum functional ability, independence, participation, and freedom from pain. Performs related duties as assigned. MAJOR JOB DUTIES AND TASKS: Percentage Of Time: 1. Patient Care 80-90% Accurately evaluate and interpret patients’ presentation in muscle strength and tone, range of motion, joint mobility, sensation, neurological systems, coordination, functional mobility skills, balance, skin integrity, edema, pain, response to treatment and patient-specific factors to determine appropriate plan of care. Establish appropriate treatment priorities with consideration of the patient’s needs and preferences. Develop and carry out effective treatment plans specifically designed for the patient, providing ongoing assessment and re-evaluation as necessary. Develop appropriate function-based treatment goals; routinely meet stated goals. Alter goals as needed. Educate patients and care partners regarding deficits, compensation, and therapeutic activities for rehabilitation. Make appropriate connections with patients, care partners, other members of the care team and payers. 2. Timely and Accurate Documentation 10-15% Complete understandable, professional, and accurate documentation that describes services provided, reflects skilled service and medical necessity, and communicates therapy assessment and recommendations. Demonstrate via peer/leader review. Record accurate billing for services provided, including using current policies and procedures for insurance and coding. Completes patient documentation, including discharge documentation, within department deadlines. 3. Professional Education and Development 5% or less Independently completes, actively participates in and maintains requirements for: Annual Required Learning Competencies – delivery of procedures, modalities and use of equipment that are standardized evaluation and treatment tools used in the physical therapy department.
