Develop Care PlansCollaborate with patients, families, and the interdisciplinary team to create and adjust individualized plans of care based on patient goals and needs
Provide Nursing CareImplement care plans, administer medications, perform nursing procedures, educate patients and families, and respond effectively to emergencies
Evaluate OutcomesMonitor patient progress, assess responses to interventions, and modify care plans to achieve desired outcomes
Communicate EffectivelyUtilize SBARQ and other communication tools to ensure safe handoffs, interdisciplinary collaboration, and accurate documentation
