About the role

Coordinate the transfer of discharge-ready patients from inpatient units to the Transition Lounge via wheelchair or stretcher.

What you’ll do

Collaborate with unit nursesCollaborate with unit nurses, Case Coordination, and support services to identify and transition appropriate patients to the lounge
Serve as a centralServe as a central resource for discharge throughput and patient flow initiatives
Provide clinical oversight forProvide clinical oversight for patients awaiting transportation or post-discharge services in the Transition Lounge
Deliver handoff communication andDeliver handoff communication and report to EMS and transportation providers to ensure continuity of care
