As a Homecare Registered Nurse (RN), you will: Provide skilled nursing care to patients in their homes Complete comprehensive patient assessments and ongoing evaluations Administer medications, treatments, and therapies as ordered by physicians Monitor patient conditions and communicate changes to the interdisciplinary care team Develop, implement, and update individualized plans of care Educate patients and family members on disease management, medications, and treatment plans Perform wound care, tracheostomy care, enteral feeding, and other skilled nursing procedures as needed Maintain accurate and timely clinical documentation Collaborate with physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes Promote patient safety, independence, and quality of life Qualifications Current Pennsylvania Registered Nurse (RN) license in good standing Current BLS/CPR Certification Minimum one year of nursing experience preferred Previous homecare, hospice, long-term care, rehabilitation, pediatric, medical-surgical, ICU, or acute care experience is a plus Strong assessment, communication, and critical-thinking skills Ability to work independently in a home setting Excellent time-management and organizational skills Reliable transportation and dependable attendance Make an Impact Where It Matters Most Homecare nurses provide more than clinical care.
