Licensure
✓Current professional license or credential in an applicable rehabilitation discipline, such as Physical Therapy or Occupational Therapy.
Qualifications
✓Experience providing rehabilitation services in home health, community-based care, or a related clinical setting
✓Strong clinical judgment, organization, communication, and problem-solving skills
✓Ability to support coordinated, outcomes-focused care across an interdisciplinary team
✓Comfort using electronic documentation and communication systems
✓Ability to travel throughout Wilmington, MA and surrounding communities as needed
✓Commitment to compassionate, client-centered care and professional excellence
Prior rehabilitation leadership, supervisory, or team-lead experience preferred
Home Health experience preferred
Experience leading or supporting PT, OT, and speech-language therapy teams (possible RNs and LPNs) preferred
Experience with care coordination, clinical documentation review, quality improvement, or regulatory compliance preferred
Experience mentoring, coaching, and developing rehabilitation clinicians preferred
Strong relationships with local healthcare providers, facilities, and community referral partners preferred
