Certifications
Certification as a Community Health Worker (CHW), Patient Navigator, or HIV specialist. preferred
Education
Bachelor's degree in Social Work, Public Health, Psychology, Human Services, Nursing, or a related field preferred.
Qualifications
✓Valid driver's license and reliable transportation may be required.
Experience with Ryan White HIV/AIDS Program services. preferred
Bilingual skills, preferred.
Knowledge of trauma-informed care, harm reduction principles, and cultural competency practices. preferred
Experience working with individuals living with HIV/AIDS, underserved populations, or in healthcare/social service settings. preferred
Knowledge of HIV disease management, treatment options, prevention strategies, and community resources. preferred
Strong communication, counseling, and interpersonal skills. preferred
Ability to build trust with diverse populations. preferred
Experience with case management and care coordination. preferred
Proficiency with electronic medical records and Microsoft Office applications. preferred
