Job OverviewThe Fountains at Hosanna! is conveniently located in Lakeville, adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments.
We are known for our exceptional staff, who create a warm, friendly, fun community. We are looking for a Resident Assistant/Home Health Aide who would provide personal cares to improve and/or maintain the senior assisted living residents’ physical and emotional well-being for our senior living in Lakeville, MN. This caregiver schedule includes;18 hours every two weeks ; Part TimeEvening Short Shift ; 4pm-10pmBlock
- Week 1: No ShiftsWeek 2: Friday , Saturday , and SundayShift differential pay availableEvery other weekend and holiday rotationResponsibilitiesPersonal Care Assistance:Supports residents with daily living activities, including:BathingFoot and fingernail careShavingDressingHair careFeedingToiletingAssistance with hearing aids and/or glasses1

- Related Support:Assists with treatments as outlined in individualized care plans under the supervision of a Registered Nurse (e.g., blood glucose monitoring).Administers or assists with medications, including insulin injections, as directed by a Registered Nurse.Takes and records vital signs such as temperature, blood pressure, pulse, and respiration.Daily Living Support:May assist with household tasks including laundry, cleaning, meal service, and resident activities.Resident Observation & Reporting:Monitors and reports changes in residents’ physical or emotional condition (e.g., mood, appetite, weight).Communicates observations to appropriate staff and contributes relevant input to care planning.Acts as a mandated reporter, promptly reporting any concerns related to potential mistreatment.Emergency Response:Responds appropriately to resident needs and building emergencies, following established protocols.Required Qualificationsless than 1 year of experienceAbility to use technology in the form of computers, handheld devices, cell phones, and fax machinesIf under 18, must be on the MN NA registryPreferred Qualifications1
