The Care Management model provides effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care utilizing performance metrics and adoption of best practices. Position Details:Required
- Graduate of an accredited School of Nursing, requiredExperience:
- 2-3 years acute care hospital setting, preferred.
- Care Management or Utilization Management experience, preferred
• Current License in the state of employment, required
• BLS (CPR) at hire date, required, or within 90 days of hire• Certification in Case Management, PreferredWhy Mercy?
