RN Community Liaison (Homecare)

Novant Health is seeking a relationship-driven professional who is passionate about building trusted partnerships with physicians, healthcare facilities, and community organizations while expanding access to home health services. In this role, you'll serve as a key ambassador, educating referral sources, strengthening strategic relationships, and ensuring patients receive the right care at the right time.

Develop and maintain strong relationships with physicians, practice groups, hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, and other referral partners to promote home health services. Serve as the primary liaison for referral sources by responding promptly to inquiries, addressing concerns, and delivering exceptional customer service. Educate healthcare providers and community partners on home health programs, patient eligibility, referral processes, and available services to increase awareness and appropriate referrals.

Collaborate with clinical and operational teams to support patient transitions of care, strengthen referral networks, and drive growth while ensuring an outstanding patient and customer experience. What We're Looking ForEducation: 4 Year / Bachelor's Degree, preferred. Graduate of an accredited school of nursing required.

Two years sales and or marketing experience in a health-related field, preferred. Licensure/Certification: Current RN licensure in appropriate state, required. Valid Driver's License in appropriate State, required.
