Full TimeShift: Monday - FridayAdventHealth will bring a 60-acre health and wellness campus to Lake Nona in late 2026. This location will offer medical innovations on a campus designed around whole person health with comprehensive services, destination lifestyle programs, and much more that will shape the next generation of health care. Our services include emergency services, urgent care, a wide range of medical specialists (including primary care), lab, imaging, orthopedics, sports medicine and rehabilitation. Acts as a liaison across the multidisciplinary team to ensure proper orders, medical records, consents, and other required documentation are received. Coordinates timely care with physicians and supportive services departments.

Maintains professional relationships with customers, patients, and team members. Provides direct patient care coordination within the Community Care and Transitional Care Programs. Educates patients, families, caregivers, and community providers to safely perform care and achieve health goals.
