RN, South Hadley Adult Medicine Practice

Baystate Health, is looking for experienced Primary Care Nurse to join South Hadley Adult Medicine Practice Full Time -40 hours Sign On Bonus eligible Full Time Monday - Friday, No weekends or holiday 4- 10 hours days 7AM- 5:30PM - Thursdays off Responsibilities Provides independent quality triage and patient care utilizing strong assessment skills, point of care testing and quality assurance checks Provides education, ongoing assessment and support to patients with chronic substance use health conditions In partnership with patient and medical staff, develop and implement plan of care for complex patients including medical and/or psychosocial disorders.

Triages and coordinates patient care ensuring access to care and patient success. Promotes health and wellness for individuals and groups of patients. Demonstrates respect and advocates for the patient’s rights, values and belief systems. ​ Required Work

• Massachusetts license as an RN and BLS CPR certified. Minimum of 2 years of nursing experience Experience providing clinical care, education, and treatment to patients with substance abuse diagnoses or primary care experience Demonstrated interest to work with patients from diverse economic and cultural backgrounds Excellent verbal and written communication and interpersonal skills. Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff Other Information Location - 470 Granby Road Full Time Monday - Friday No weekends or holiday 4- 10 hours days 7AM- 5:30PM - Thursdays off Sign On Bonus eligible- Must have 1 year of outpatient experience Sign on Bonuses are paid to external hires only; not applicable for internal or company transfers, unless otherwise specified.
