Qualifications
✓5 years of Registered Nursing experience, excellent communication skills, basic computer skills, strong work ethics and accountability.
✓Review symptoms, medical history, medications, and vital signs reported by patients.
✓Provide nursing advice, education, and self-care instructions.
✓Triage patients and determine the appropriate level of care.
✓Coordinate appointments, referrals, labs, and follow-up care.
✓Monitor chronic conditions and follow up with patients.
✓Communicate with physicians and other healthcare providers.
✓Document assessments, interventions, and patient communications in the EHR.
✓Follow clinical protocols, privacy requirements, and telehealth policies.
✓Identify urgent or emergency symptoms and direct patients to appropriate in-person or emergency care.
✓Make follow-up calls and participate in care-management programs, depending on the position.
✓This position is subject to pre-employment screening (criminal background, drug testing, and/or education verification).
