About the role

This is an exciting opportunity for an RN who enjoys building long-term relationships with patients, coordinating complex care, and working as part of a highly collaborative transplant team. This is a full-time, 80 hours per pay period, benefit-eligible, exempt position offering a flexible hybrid schedule. Team members work onsite two days per week and from home three days per week.

What you’ll do
Coordinate patient care inCoordinate patient care in the post-transplant phases
Collaborate with interdisciplinary teamCollaborate with interdisciplinary team members within the organization and outside community partners
Act as a liaisonAct as a liaison for transplant physicians to implement medical management of conditions
Provide telephone triage andProvide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes
