Day shift | PRN How you’ll make an impact in this role Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values (LSW required, LCSW preferred). Monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services. Advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care.
Maintain ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.
