Saint Luke's South Hospital is seeking a full-time Social Worker to join our Team! Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning.
Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.
Saturday and Sunday7AM-7PM hybrid, but heavily remote. Provides social work services to patients in an acute care setting for patients with acute, chronic or terminal illnesses and their families. Services to patients and their families include: identification of psychosocial needs, psychosocial assessment, discharge planning, crisis intervention, information and referrals, protective services, and patient/family educationFunctions as a member of a multidisciplinary team for patient careDuties may also include assistance with program development, evaluation, professional education, or supervision of students and BSW staff.
Knowledge of theory and practice of social work at the MSW level and have basic research skillsMust have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgroundsMust have excellent written/verbal communication skillsCertified LMSW or LCSW requiredKansas Social Work license requiredMaster's Degree in Social Work requiredPrefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.
