The Social Worker will provide clinical and case management care to patients with intellectual and or developmental disabilities across the life span in an outpatient setting. The clinician will also work with children ages 0-5 in our Early Childhood program. Documentation must be in a timely and quality manner.

Conduct individual, group, and family therapy. Assess, identify, and provide services for patients/families; address psychosocial concerns as well as post hospital care, including crisis intervention, advocacy and community referrals.

Master's degree required. Licenses/Certifications:NYS Registration/Certification as a Social Worker (LMSW/LCSW) is required. Experiences:2 yrs post-graduate work preferred.

Prior experience in a hospital or community health agency is preferred. Experience with the assigned patient population is required. Additional language fluency and computer literacy preferred.

Knowledge, Skills, & Abilities:Clinical Assessment & Treatment: Skilled in psychosocial assessments and delivering evidence-based therapy. Crisis Management: Able to intervene in mental health or situational crises. Care Coordination: Experienced in connecting clients to services and managing treatment plans.

Cultural Competence & Advocacy: Committed to equitable, client-centered care across diverse populations. Professional Communication: Strong documentation, collaboration, and interpersonal skills. Ethics & Compliance: Knowledge of HIPAA, NASW Code of Ethics, and legal reporting requirements.

Mental Health Expertise: Understanding of DSM-5, trauma-informed care, and co-occurring disorders. Work
