Qualifications
✓At least one year of social work experience in a healthcare setting
✓Strong counseling, communication, documentation, and advocacy skills
✓Knowledge of community programs and available patient resources
✓Ability to work independently in patients’ homes
✓Willingness to travel throughout the assigned service area with 50% travel required
✓Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
✓Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
