Care Coordination Supervisor Position Summary The Peninsula Regional Clinically Integrated Network (PRCIN) Care Coordination Supervisor, in collaboration with organizational leadership is responsible and accountable for the planning, developing, implementing, evaluating and monitoring, of policies and procedures, budgets, and allocation of resources for care coordination and care management programs.
The Care Coordination Supervisor will provide supervisory oversight to the clinical and non-clinical care coordination/care management staff, to include training and competency development of team members, establishing standard work for all roles, and performing ongoing audits and assessments. Additionally, the Care Coordination Supervisor will, as needed, staff the various care coordination services to ensure operational service excellence.
Care Coordination Supervisor Position Requirements Education RN with BSN, BS in related field. Experience Three + years’ experience in case management, care coordination, disease management, ambulatory care, physician group or professional practice, managed health plans, or combination preferred Five + years’ experience in clinical nurse role. One-year supervisory experience.
Experience with Epic electronic health record preferred Licensure/Certification Licensure as RN in the State in which you live required. Care Management certification preferred. Care Coordination Supervisor Benefits At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
