Team Based Care Clinic RN

The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and documented workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care.

Coursework/Training: Graduate from an accredited school of Nursing. California Registered Nurse License upon hire Two (2) years Professional Nursing Experience.

Bachelor's Degree in Nursing. One (1) year experience in ambulatory care nursing.
