You’ll need
Licensure
✓Registered Pharmacist License.
Board Certified Pharmacotherapy Specialist. preferred
Certifications
Medication Therapy Management Certification. preferred
Education
✓Bachelor's Degree in Pharmacy or PharmD from an ACPE accredited pharmacy school.
Qualifications
✓Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing in geriatric and adult populations.
✓Must be self-directed and possess excellent organizational and communication skills.
✓Excellent written and verbal communication skills.
✓Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.
✓Ability to appropriately treat private and confidential information as required by HIPAA.
A minimum of one year of work experience, preferably in an acute care setting. preferred
Two years of work experience. preferred
Pharmacy Residency (PGY-1). preferred
Performance of the 5 Core Elements of Medication Therapy Management Services: preferred
Comprehensive Medication Therapy Review (CMR) preferred
Personal Medication Record (PMR) preferred
Medication-Related Action Plan preferred
Interventions and Referrals preferred
Documentation of Services and Follow-Up preferred
Demonstrates excellent listening skills, and maintains empathy with callers in tone, language, questions and counseling. Able to employ the teach back method during patient/family counseling. preferred
Ability to probe and question patients and providers to extract information to understand concerns and needs. preferred
Demonstrates professional conduct in dealing with healthcare providers, patients and/or caregivers at all times. preferred

About the role
The Transitions of Care Pharmacist is responsible for involvement in the clinical aspects of pharmaceutical patient care including interaction with patients, families, physicians, nurses, and other healthcare providers.
