MedStar Health is looking for a Vocational Specialist to join our HEART Team! We are looking for someone with personal stability, flexibility, energy, and commitment to working with people with medical and behavioral health disorders, excellent oral communication, writing, organizational skills, and abilities. The primary responsibility of the Vocational Specialist- HEART Team is to provide vocational support services to beneficiaries on the HEART Team.
The HEART Team Service is a holistic, interdisciplinary, community based medical and behavioral health service designed to help beneficiaries manage complex medical and behavioral health illnesses. It aims to restore health, improve community, daily living, personal, and social skills while reducing incidence of medical or behavioral health crises that lead to unnecessary use of the hospital system. Apply today and learn how MedStar Health can be your next great career move! Primary Duties: Provides vocational counseling and coaching support to a caseload of beneficiaries to address barriers to employment. Conducts evaluations using approved assessment to determine vocational support needs.
Participates in regularly scheduled caseload consultation; includes all disciplines. Includes patient and family in mutual goal setting and recommended care plan revision. Is responsive to and communicates respectfully with patients/families, HEART Team members, and other health team members.
Conducts regular individual meetings with beneficiaries to review progress toward vocational goals. Assists with and tracks competitive employment opportunities. As necessary, coordinates and schedules functional capacity evaluations.
Develops plans for job seeking skills training as needed. Assists with the job application process. Explores and coordinates transportation resources as needed.
Acts as a liaison between beneficiaries, external resources, and employers as needed. Builds relationships with area employers. Develops employment opportunities for beneficiaries through direct solicitation of employers.
Provides beneficiaries with ongoing support for employment retention. Provides services remotely, in-office, telephonically, via telehealth, in the community, or beneficiary's home as beneficiary needs and preferences dictate. Drives and travels to community locations, various agencies, and other outreach destinations.
Maintains confidentiality and adheres to HIPPA law and Maryland State Regulations.
Bachelor's degree in human services. Professional experience in a behavioral or social science related field. 1-2 years' experience with career counseling. Driver License, Valid and in State.
This position has a hiring range of USD $23.65 - USD $42.03 /Hr.
