Navigates and coordinates the care of patients across the continuum of care. Monitors and manages resources to transition patients to the appropriate level of care in a cost-effective fashion while ensuring quality outcomes and patient satisfaction. Assesses, plans, coordinates, monitors and evaluates the patients’ plan of care in collaboration with the healthcare team.
Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educator payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.

The areas are air conditioned as well as heated. Not exposed to environmental or toxic hazards. PERSONAL PROTECTIVE EQUIPMENT:Universal Blood & Body Fluid Precautions & Isolation restrictions are observed and appropriate protective equipment utilized.
