Care Manager (RN)

Department: Managed Long Term Care / Care Management · Schedule: 35 hours/week | 7 hours/day | 5 days/week

We are seeking an experienced Registered Nurse Care Manager to support care coordination and facilitate high-quality, member-centered care. The Care Manager will work collaboratively with members, families, physicians, nurses, and other healthcare providers to develop and monitor care plans, coordinate services, identify clinical risks, and support members across the continuum of care.
