The Case Manager is responsible for managing the healthcare needs, facilitating the achievement of quality and cost outcomes, and coordinating services for all patients. This role involves working closely with a multidisciplinary team to ensure that patients receive appropriate care throughout their stay. Key Responsibilities Manage patient healthcare needs and coordinate services across the continuum of care.
Negotiate, procure, coordinate, and monitor necessary resources for patients. Conduct patient reviews at admission and throughout their hospital stay to assess care levels and resource utilization. Implement transition care planning activities to secure appropriate post-hospital arrangements.
Participate in performance improvement initiatives. Additional Requirements Strong organizational skills and the ability to analyze and present data. Proficient in computer literacy and data management.
Excellent interpersonal and communication skills. Ability to maintain confidentiality. Collaboration and negotiation skills with various stakeholders including providers, patients, and insurance organizations.
Ability to work independently and manage multiple open applications. Strong knowledge of local community resources and regulatory organizations. Familiarity with Medicare, Medicaid, and third-party payers required.
Experience in data comprehension, research, analysis, interpretation, and judgment for reporting purposes. Qualifications 3+ years of clinical experience in acute care settings such as internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology. Experience in rehab settings, home health, or long-term care is considered.
Ability to conduct comprehensive admission and discharge risk assessments. Preferred
Certified Case Manager or Accredited Case Manager. Preferred RN-BC certification in Case Management. Experience with Meditech Expanse is highly preferred.
Must possess a valid Massachusetts RN license. Current American Heart Association or American Red Cross Basic Life Support (BLS) certification required.
