Days | 9:00 AM – 5:00 PM | Monday – Friday (37.5 hours/week, alternating weekends and holidays)Position OverviewWe are seeking an experienced, detail-oriented Case Management Registered Nurse (RN) for a permanent direct-hire role with an award-winning hospital system in Westchester County, NY. In this role, you will perform comprehensive assessments upon admission, identify high-risk patient populations, coordinate complex discharge planning, and ensure seamless continuity of care to prevent avoidable readmissions.
Key ResponsibilitiesConduct initial comprehensive clinical assessments and admission screenings using standardized high-risk identification tools. Formulate, coordinate, and execute individualized discharge plans in multidisciplinary collaboration with physicians, social workers, and nursing staff. Monitor resource utilization, assess post-acute care needs (SNF, home care, durable medical equipment), and support smooth patient transitions.
Provide ongoing reassessments throughout the patient's hospital stay to optimize clinical outcomes and mitigate readmission risks. Qualifications & RequirementsLicensure: Active, unencumbered New York State (NYS) Registered Nurse license.
Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) is a plus.
Minimum of 1–3 years of dedicated Inpatient Case Management / Care Coordination experience in an acute care hospital setting.
PRI / NYS SCREEN certification preferred (or willingness to obtain within initial employment period); BLS preferred. Work History: Strong, stable clinical background with documented work history longevity. Immunizations: Annual Flu vaccination required (mandatory hospital policy).
