Qualifications
✓2+ years of recent acute-care RN Case Management experience
✓Recent Care Coordination experience is required and must be within the last 3 months
✓Patient assessment
✓Discharge planning
✓Treatment planning
✓Family meetings
✓HMO/IPA coordination
✓DME, SNF and ambulance arrangements
✓Knowledge of resources related to mental health, homelessness, substance abuse, advance directives/POLST, protective services, adoption/foster care, etc
✓Must be able to independently arrange DME, SNF and ambulance transportation
