✓Minimum 5 years of acute adult care nursing experience
✓Discharge planning and care coordination experience
✓Must float to meet Care Management department needs
✓Patient ratio: 1:20–24
✓Patient population: High-acuity, complex patients with a high percentage of behavioral health cases
✓EMR/systems: Epic, WellSky, CarePort, PASSR, Microsoft Teams
✓Equipment: Laptop and Ascom phone
✓Dress code: Business casual or professional casual
✓Scrubs acceptable
✓Call: None
✓Guaranteed Hours: 36/week
