Day shift M-F Contract Length: 13 weeks Candidate Type: Traveler

WV or compact license; HH experience; OASIS; HCHB experience; Associates or Bachelor’s Degree in Nursing or related field; current, unrestricted state Registered Nurse license; ability to analyze complex medical information and make rational decisions; Case Management certification

Perform comprehensive assessments of high-risk members. Collaborate with primary care providers to implement individualized, comprehensive care plans. Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
