Contract details

Under the direction of the Director of Primary Care, provides services to psychosocial evaluation, crisis and trauma counseling, resource referral and case management services. Accountable for intervening with patients and families, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression an transition/discharge plan for patients, including facilitation of patient and family communication, coping and decision making.

Works as part of an interdisciplinary team. Education and ExperienceMinimum Required:Master’s degree in social work from an accredited program with licensure as Licensed Clinical Social Worker (LCSW) in New York State. Experience working with specific populations, such as individuals with mental health concerns, families, or older adults.

Experience with electronic health records, ability to work independently and as part of a team and commitment to ethical and culturally sensitive practice. Preferred: Preferred 3 years of postgraduate experience. License and CertificationMinimum Required:NYS LCSW required.

Work EnvironmentWork is performed in an office environment and patient care areas which require working in proximity to other employees and patients. Rooms are air conditioned and heated. If this is strictly an off-site telehealth position, the off-site location should be neat professional and ensure lighting is sufficient so that patients can easily see the face of the LCSW.

Camera, microphone and computer must meet DVH IT department standards to ensure clean communication with the patients. Frequent walking, sitting, standing, bending and lifting. Fast paced environment with frequent interruptions.

PPE (Eyewear, Masks, Face shields, Shoe covers, and Gloves) as required by UHS Hospitals Infection Control policies. Employees are required to have reasonable access to transportation to meet work requirements made in the ordinary course of business in a timely and efficient manner.
