Physical Therapist - Home Health

Travel PT Home Health jobs in Manhattan, NY are available starting November 2, 2026, for a 13-week contract. You will work Monday through Friday, 8-hour days, seeing 5 to 6 geriatric clients daily in a walkable territory using public transit. You must have an active New York PT license and experience with the geriatric population.

Home health experience is preferred but not required. Limited time off is a must. Responsibilities include conducting in-home assessments, developing individualized care plans, providing therapeutic exercises, and documenting care using Homecare Home Base.
