Details Home health setting covering the Paducah territory, including Mayfield, Wickfield, Salem, Benton, Arlington, and areas near LY Lake and Lake Barley Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM Average patient load of 5-7 patients per day Patient population consists of geriatric cases No on-call requirements Territory covers up to a 60-mile radius from the hub office Requirements Minimum 1 year of recent Physical Therapy experience required BLS certification required OASIS experience required HCHB EMR experience required License required if applicable Additional Information Timely communication and documentation required Dress code is scrubs in any color, no jeans permitted iPad provided for documentation Mileage reimbursed at the IRS standard rate Accepts local candidates at the travel rate
