SimpliFi Interview & Manager Second Interview for Offer ? Unit Information: o Beds: 22 o Minimum Years of Experience required? 6 months of children/adolescent experience o Will you accept a first-time traveler? No, travelers must have completed at least one travel assignment with the agency that is submitting them ?

Patient Types/Common Diagnoses: o Children typically age 3-10 with intellectual and Developmental Disabilities. These patients can be impulsive and require redirection o Can range from verbal to nonverbal (full spectrum). Some may need full ADL's, some need to change them or assistance.

Some will have communication issues o May need full support of ADL's but may need prompting of good hygiene o Typically don't need to feed but may need to be prompted o Patients have been known to kick, hit, grab, yell o Very busy unit, multiple physical holds for safety when redirection ineffective o State guardians frequently (difficult placements) o ? Patient Ratios: o Typically, 2 RNs per unit o 2-3 MHTs ? Required License, National Certification, Certs: o BLS, CPI obtained on site (physical intervention- must be able to transition from standing, seated and supine for safety of patients) ?

Support within the Department: o Leadership is highly supportive of employee safety and willing to condense units or implement operational changes as needed to maintain safety. o INBC Team– Innovation Specialist, Behavioral Analyst o Regular rounding from Advanced Practitioners, clear call sheet to page o MHT (2 is desire but not guaranteed) ? Support within the facility o Therapist o Case Managers o Security on campus o Dedicated code response resources include transponders, five designated response team members, security support, and 24/7 video monitoring staff who can initiate assistance or call codes from off-unit. ?

Technology/Equipment: o EMR: Cerner o IV Pump: N/A o Med dispensing: Omnicell o Unit specific equipment: Restraint chair (rare to use), physical holds, timeouts, seclusion ? Floating: If yes, where? o Yes, could float to children to adults, anywhere through facility. o Any age could be present on unit, depending on how patients are needed to be separated for that shift o Candidates should be comfortable floating throughout the unit and open to schedule/shift flexibility, understanding they may not remain on the same unit or shift consistently. ?

Orientation (facility and unit): o NEO o 3 12h shifts, 1 12h shift float to other units ? Scheduling: o Weekend rotation: Every third weekend o On call? No o Holiday Expectations: Based on scheduled days from facility o Approval of RTO: 5 days
