Louisville, KY 40205 o 261 Beds Adult, Adolescent, and Pediatric Behavioral Health facility o Patient population can be highly challenging; some adolescents have extended stays (300+ days) and may display high-risk or hopeless mentalities. o Average length of stay is approximately 12 days, though there are a fair number of long-term patients staying 100+ days.
Unit Information: o Beds: 22 o Minimum Years of Experience required? 6 months o Will you accept a first-time traveler? No, travelers must have completed at least one travel assignment with the agency that is submitting them Patient Types/Common Diagnoses: o Adult patients o Depression, anxiety, lower functioning who are under state guardianship, hold for placement for long term stays o Potential for aggressive patients Patient Ratios: o Desired- 2 RNs on the unit, Minimum- 1 RN on unit (more common currently-please note in interview) o 2-3 MHTs desired, but not guaranteed Required License, National Certification, Certs: o BLS, CPI obtained on site (physical intervention program, requires transitioning patient from standing to seated, seated to supine) Skills required: o Soft
Patience, compassion, not taking things from patients personally o Comfortable with physical holds is a must, verbal deescalation the gold standard, seclusion and or mechanical restraints if indicated for safety o IM injections o Recognizing increased agitation proactively o Manage multiple escalated patients at the same time Support within the Department: o Program Coordinator o Leadership is highly supportive of employee safety and willing to condense units or implement operational changes as needed to maintain safety. o INBC Team- Innovation Specialist, Behavioral Analyst o Regular rounding from Advanced Practitioners, clear call sheet to page o MHT (2 is desire but not guaranteed) Support within the facility o Therapist o Case Managers o Security on campus o Dedicated code response resources include transponders, five designated response team members, security support, and 24/7 video monitoring staff who can initiate assistance or call codes from off-unit.
Technology/Equipment: o EMR: Cerner o IV Pump: N/A o Med dispensing: Omnicell o Unit specific equipment: restraint bed, restraint chair, new windows, video monitors (not on unit or viewable by unit staff). Nurses station enclosed. Floating: If yes, where? o Yes, could float to children to adults, anywhere through facility. o Any age could be present on unit, depending on how patients are needed to be separated for that shift o Candidates should be comfortable floating throughout the unit and open to schedule/shift flexibility, understanding they may not remain on the same unit or shift consistently.
