Case Manager - Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med. POSITION DUTIES Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/team. Need to have a positive attitude, be able to critically think, and good time management skills.
