Doctor of Physical Therapy (DPT) from a program registered by New York State Education Department or accredited by CAPTE, or equivalent if educated outside the USA. Pass the National Therapy exam (NPTE) and be licensed to practice Physical Therapy in NYS. Minimum one year experience.

Candidates that meet minimum requirements should be submitted with resumes. Schedule and anticipate length of assignment: 20 hours per week, Monday to Friday. 8am to 4:30pm, hours are flexible. Background check

Influenza (during season), Hepatitis B and Covid-19. OMH required training (This training will be provided by OMH): TRUST and annual standard trainings. Job Duties:Perform initial evaluation and functional assessment of newly admitted patients and patients returning from outside hospitalization.

Develop individualized treatment plan based on clinical findings. Provide skilled physical therapy interventions for patients. Monitor progress and modify interventions as clinically indicated.

Document in EHR timely and accurately all evaluations, treatment, progress notes and discharge summaries. Educate patients and direct care staff regarding diagnosis, treatment plans, fall and injury prevention strategies. Collaborate with physicians and other team members to ensure coordinated care for patients.

Serve in Safe Patient Handling and Falls committees as needed. This is not an exhaustive list.
