As a Clinical Pharmacist, you will serve as a trusted medication expert and integral member of the healthcare team by:Reviewing and evaluating medication therapies for safety, effectiveness, and appropriatenessCollaborating with physicians, nurses, and other clinicians to optimize patient careIdentifying and resolving medication-related issues, adverse reactions, and drug interactionsMonitoring patient responses to therapy and recommending adjustments when appropriateProviding medication education and consultation to patients and healthcare providersServing as a key clinical resource on formulary management, medication utilization, and evidence-based practicesParticipating in quality improvement initiatives, medication-use evaluations, and patient safety programsSupporting pharmacy learners through student and resident education opportunitiesMaintaining accurate clinical documentation and utilizing technology systems including Epic, Micromedex, and pharmacy automation platformsWhy Mercy?
