The Clinical Pharmacist I collaborates with an interdisciplinary healthcare team to ensure safe, effective, and evidence-based medication therapy. This role serves as a key clinical resource for physicians, nurses, pharmacy staff, and patients, while supporting high-quality patient care aligned with organizational mission, values, and service standards. Key ResponsibilitiesClinical Practice & Patient CareReview patient records to evaluate the appropriateness and effectiveness of medication therapyAssess patient conditions to ensure comprehensive treatment of all clinical concernsDevelop and optimize pharmacotherapy plans to improve outcomes and minimize adverse effectsMonitor drug therapy through chart review and interdisciplinary collaboration; intervene when appropriateConsult with providers, nurses, and patients on medication selection, dosing, administration, interactions, and side effectsEvaluate treatment outcomes and adjust therapy recommendations as neededMaintain accurate and timely documentation of patient care activitiesProvide Medication Therapy Management (MTM) services, including dose, route, schedule, contraindication, and interaction assessmentCollaboration & CommunicationWork closely with physicians, nurses, and allied health professionals to support patient-centered careServe as a drug information resource for clinical staff and patientsMaintain strong professional relationships across care teamsCommunicate effectively across shifts to ensure continuity of careOperational & Quality ResponsibilitiesEvaluate medication orders for clinical appropriateness and operational efficiencyParticipate in quality assurance initiatives, including adverse drug reaction reporting and medication use evaluations (MUEs)Assist with departmental initiatives such as policy development and reviewEnsure compliance with regulatory, licensing, and accreditation standardsParticipate in investigational drug protocols as assignedEducation & Professional DevelopmentPrecept pharmacy students and residents; support training and competency developmentProvide in-services, presentations, and continuing education to staffMaintain current licensure and continuing education requirementsParticipate in committees and professional development activities as neededServe as a clinical resource and maintain expertise within designated specialty areasSafety & ComplianceAdhere to all confidentiality and patient privacy policiesComplete required safety training, inspections, and compliance documentation (e.g., fire/safety, expiration checks, floor stock reviews)Uphold infection control standards, PPE requirements, and organizational policiesTechnology & SystemsDemonstrate proficiency with pharmacy systems (e.g., Epic) for order verification and clinical documentationUtilize drug information databases (e.g., Micromedex) effectivelyOperate pharmacy automation systems (e.g., Omnicell, ScriptPro, McKesson, Baxa) as applicableProfessional StandardsDeliver exceptional service using organizational communication and customer service standardsDemonstrate professionalism, teamwork, and accountability in all interactionsMaintain professional appearance and adhere to dress code guidelinesEffectively manage time and prioritize responsibilities in a fast-paced environmentQualificationsEducation:Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school of pharmacyLicensure:Active pharmacist license in the state of practiceExperience:1–4 years of clinical pharmacy experience preferredWhy Join Us?
