JOB SUMMARYThis position is part of the acute care pharmacy department and functions as the pharmacy expert on the interdisciplinary Internal Medicine team.

The IM specialist serves as a leader for the department in their specialty area of practice through organizing and executing appropriate departmental initiatives, assisting with quality initiatives and practice development in the IM service lines, and providing education and teaching for pharmacy colleagues, providers, patients, and trainees at all levels of practice. Day-to-day activities include rounding on the inpatient Internal Medicine service; evaluating, monitoring, and modifying drug therapy to optimize patient outcomes and safety; and precepting pharmacy residents and/or students.

Non-direct patient care activities may include cost savings initiatives, committee involvement, and participation in departmental projects and practice advancement efforts. EXPERIENCE AND EDUCATIONRequiredEducationBoth PGY1 and PGY2 from an ASHP-accredited residency programPGY1 with 5 years of progressive clinical practice experienceBachelor's Degree of Science degree or PharmD from ACPE accredited school of pharmacy orDoctor of Pharmacy (PharmD) degree from an ACPE accredited school of pharmacy Experience5 years progressive clinical practice experience within specialty with PGY1 from an ASHP-accredited residency program or8 years progressive clinical practice experience within specialty.

Computer skills, data entry experienceEnglish (speaking, reading, writing). Good general health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc.

Licenses and Certifications(BLS) BASIC LIFE SUPPORT certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire and(TSBP) REG PHARMACY TECNICIAN Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required Upon Hire and(TSBP) REG PHARMACY TECNICIAN - Texas license with TSBP within 30 days of date of hire within 30 Days(ACLS) ADV CARDIAC LIFE SUPPORT course accredited by the American Heat Association (AHA), or American Red Cross (ARC) required for only inpatient positions prior to or within 6 months of employment.

Upon HireJOB DUTIESPerforms job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards. Develops/implements process to meet/comply with JCAHO and regulatory requirements. Coordinates staff training, competencies, certification and continued education specific to specialty.

Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, TPN assessment and pharmacokinetic calculations. Identifies opportunities for process improvements, develops and implements policy and procedures for prompt, safe and efficient patient care. Actively promotes pharmacy.

Coordinates and participates in multidisciplinary in-services/meetings, publishes articles (i.e. staff newsletter), individualized drug information, rounds, consults, etc. Maintains high visibility/availability through participation in in-services, meetings and daily activities. Participates in the education/experiential training of pharmacy students and pharmacy residents.

Actively manage P&T initiatives through monitoring of documented interventions. Verifies data, develops measures and reports outcomes (Performance Improvement). Supports cost savings initiatives.

Has active role in assessment and selection of cost-effective pharmaceuticals. Reviews formulary addition request and develops monographs for presentation. Manage the formulary system and procedures for acquisition of non-formulary product.

Consistently works and problem solves independently and effectively. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment.

Cross trained and multifunctional maintaining competency in multiple areas. Mentors clinical staff pharmacists and supportive personnel. Delegates appropriate/reasonable tasks and evaluates performance.

Represents Pharmacy Department at P&T Committee and Service Line Meetings. Performs computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges.

Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (i.e., ADRs). Actively enhances personal growth and development by participating in educational programs, literature searches, in-services and meetings.

Actively participates in new employee orientation and training and evaluations. Duties performed may include one or more of the following core functions: a) Directly interacting with or caring for patients; b) Directly interacting with or caring for human-subjects research participants; c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.

Takes ownership in solving problems effectively, efficiently and to the satisfaction of the patientPresents constructive feedback regarding system or process failures and participates in resolution in a proactive and positive manner. Maintains patient confidentiality in accordance with University Medical Center policy. Consistently maintains control of feelings, emotions and body language.
