The LPN works under the direction of a Registered Nurse to deliver patient-centered care, support treatment plans, monitor patient conditions, and assist with admissions, discharges, and patient education.
Position Details:Key ResponsibilitiesProvide direct patient care within the LPN scope of practice. Assist with patient admissions, discharges, assessments, and care planning. Monitor patient conditions and promptly communicate changes to the RN.
Participate in bedside shift reports, patient rounding, and interdisciplinary collaboration. Reinforce patient and family education. Delegate and support patient care activities for unlicensed assistive personnel as appropriate.
Maintain accurate documentation and follow hospital policies and procedures. Complete required competencies, certifications, and ongoing education. Contribute to quality improvement initiatives and department goals.
QualificationsGraduate of an accredited Practical Nursing program. Current LPN license in the state of practice. New graduates and experienced LPNs encouraged to apply.
Strong communication, teamwork, and patient care skills. CertificationsBLS (American Heart Association) required within 30 days of hire. IV Certification required upon hire or within 3 months of hire, based on department requirements.
Additional certifications (ACLS, PALS, PEARS, S. T. A.
B. L. E., or other unit-specific certifications) may be required based on assignment.
